Osteonecrosis of the femoral head remains a difficult management problem for the orthopedic surgeon. The fact that this condition is, seen in a heterogeneous group of patients representing a variety of disease entities with a spectrum of severity makes treatment particularly challenging. Because patients are often young and active. the selection of the proper treatment modality is essential for long·term hip function and durability. Treatment options designed to retain the femoral head have included the following: protected weight bearing. '-1 core decompression with or .without bone-graft augmentation, muscle-pedicle-grafts. vascularized bone-grafts and osteochondral-grafts. 1O rotational or angular osteotomies. and pulsed electromagnetic fields.
